Synopsis: In this retrospective chart review, researchers compared patient outcomes for laser resurfacing in 665 patients with phototypes III-V who received CO 2 ablative laser, CO2 microfractionated, or both. They found that most patients (80%) received ablative laser, and the ablative group also had the highest hyperpigmentation rates (30% vs 11% and 16% respectively). They also observed a steady increase in hyperpigmentation rate with increase in skin phototype, although they concluded that staged treatments and adequate post- procedure care can reduce complications. Synopsis: Since most published data on outcomes of laser and light treatments for hair removal and resurfacing in nonwhite skin involve individuals of East Asian ethnicity, here the author reviews laser and light-based modalities specifically for individuals of African ancestry and/or with Fitzpatrick skin phototypes V or VI. Such modalities include longer wavelength lasers, intense pulsed light and monochromatic excimer light, fractional lasers, and radiofrequency devices. Synopsis: To study whether wound healing improves after fractional ablative laser when immediately followed by laser-assisted delivery of vitamins C and E and the phenolic phytochemical ferulic acid, researchers conducted a double blinded, prospective, single center, randomized split face trial with 15 healthy adults. They found increased expression of basic fibroblast growth factor and decreased edema and erythema on the treatment sides, and concluded that immediate delivery of the study topical correlated with more rapid wound healing after fractional ablative laser. PMID: 26612341 Improving the outcome of fractional CO 2 laser resurfacing using a probiotic skin cream: preliminary clinical evaluation. Zoccali G, Cinque B, La Torre C, et al. Lasers Med Sci. 2016;31(8):1607–1611. Epub 2016 Jul 13. Synopsis: Here, authors report their experience using a post-operative probiotic cream to reduce redness and swelling after fractional CO 2 resurfacing in 42 consecutive patients, compared to 20 controls treated with an antibiotic cream and a hyaluronic acid based cream. They found that the treatment group averaged 14 days for erythema resolution and 9 days for swelling resolution compared to 24 days and 16 days, respectively, for the control group. The authors concluded that the study treatment reduced postoperative erythema and swelling more quickly compared to standard treatment. PMID: 27412287 Topical corticosteroids minimise the risk of postinflammatory hyper- pigmentation after ablative fractional CO 2 laser resurfacing in Asians. Cheyasak N, Manuskiatti W, Maneeprasopchoke P, Wanitphakdeedecha R. Acta Derm Venereol. 2015;95(2):201–205. Synopsis: To explore whether a topical corticosteroid reduces post-inflammatory hyperpigmentation after ablative fractional resurfacing in Asians (skin phototype IV), the authors conducted a split face analysis in 40 individuals with atrophic acne scars. After receiving fractional CO2 laser on both sides of the face, they received clobetasol propionate 0.05% ointment on one face side and petrolatum jelly on the other. The authors found significantly lower incidence, lower intensity, and smaller area of post- inflammatory hyperpigmentation on the treatment side compared to control, and concluded that postoperative topical corticosteroid is associated with a decreased risk of post-inflammatory hyperpigmentation after ablative fractional resurfacing.
